Student Athlete Forms
All forms must be completed in full each year and received by the Athletic's staff prior to your first practice with an intercollegiate team. The form can be mailed and must be received no later than August 2nd if your first practice takes place before the start of classes for the fall semester. For all other athletes, please return all forms prior to the start of your practice season.
