MEDICAL REQUIREMENTS FOR RETURNING STUDENT-ATHLETES

RETURNING STUDENT ATHLETES - must complete and submit the following forms:

FOR ALL FORMS THAT ARE SHADED, PLEASE TYPE ALL INFORMATION IN THE BOXES PROVIDED

• Fitchburg State College Athletic Training Pre-Participation Health Questionnaire.
• Insurance Information - Memorandum, Acknowledgement, Insurance Form (Complete all three).  
• If you are taking a medication for ADD/ADHD then you must read the following:
  NCAA Medical Exceptions Policy and submit the requested paperwork.
• Concussion Information - Concussion Statement and Agreement

Please make sure that your tetanus shot is up to date (within 10 years). We may require you to be cleared by the team physician for any previous injuries or for those that occurred since you last participated.

Football Student-Athletes Only: you must read, sign, and return the Football Helmet Warning Form.

Please be prepared to fill out all medical information including your physician's name and phone number, dentist's name and phone number and insurance information which is on your insurance card.

As soon as all information is submitted, you will be able to participate.  We look forward to another exciting season of Falcon Athletics!
